Middle School Athletics

2025 FALL SPORTS INFORMATION

Athletic physicals and required signatures must be completed before a student-athlete is allowed to practice for any sport. Student-athletes should register on Bound to submit paperwork and complete required forms. 

The following items are required:

• Medical Eligibility form signed and dated by a qualified health care professional (valid for 13 months after exam date) 

• Completed signature forms: concussion, insurance, and code of conduct

These forms are available on Bound or in the school office. New signature forms must be submitted each school year.

Cross Country

Please email Coach Fober ASAP if you would like to participate in middle school cross country.
